Hundreds of Slavia fans stormed the pitch seven minutes into stoppage time of Saturday's game against arch-rivals Sparta Prague, firing flares into the Sparta stands and attacking players.
Slavia were leading 3-2 and on track to secure a second consecutive title as the win would have given them an unbeatable 11-point lead ahead of Sparta with three games to play.
But referees abandoned the game as players were rushed off the pitch and Sparta then refused to return to play the final three minutes.
"Slavia Prague as the hosts were fined 10 million koruna ($480,000)," the highest possible fine, said Jiri Matzner, head of the LFA's disciplinary committee.
"Since the match could not be finished because of security concerns, the disciplinary committee decided to hand the home side a 3-0 defeat by forfeit," he added on X.
The defeat has narrowed Slavia's lead to five points.
"At the same time, Slavia were sanctioned by having to play the next four competitive games behind closed doors," Matzner said.
The mob attacked in particular Sparta's Slovak goalkeeper Jakub Surovcik who was visibly shaken after a fan splashed beer in his face.
Sparta were also fined 600,000-koruna ($29,000) after their fans threw flares onto the pitch and damaged the stadium.
