The champions started brightly and nearly broke the deadlock within the opening five minutes, but Mwapheku and Hassan Athuman both squandered clear opportunities.
Khadija responded with a series of attacks of their own, but it was Tononoka who struck first when Mwapheku rose highest to head home a perfectly delivered cross from Fadhili Adinani.
The advantage was doubled in first-half stoppage time as midfielder Bakari Hamisi unleashed a spectacular 30-yard effort that caught Khadija goalkeeper Masoud off his line, sending Tononoka into the break with a comfortable 2-0 lead.
Determined to fight back, Khadija piled on the pressure after the restart but failed to convert their chances. Their best opportunity came in the 61st minute when Ali Said missed a golden chance to reduce the deficit.
With 10 minutes remaining, Said again came close but failed to connect with a dangerous corner from close range, as Tononoka's defence continued to stand firm.
Mwapheku put the result beyond doubt in the 88th minute, calmly beating the Khadija goalkeeper after latching onto a well-weighted pass from Suleiman Mohammed to complete his brace and cap off a dominant 3-0 victory.
Speaking after the match, Tononoka head coach Juma Kalato hailed his players for ending the school's long wait for county glory.
"I'm happy for my boys and the Tononoka staff for making this possible. It was a hard journey, though we have won it unbeaten from the sub-county level. I know the regional level will be tough, but we are ready."
Despite the defeat, Khadija Secondary head coach Shem Lumumba remained optimistic ahead of the regional championships.
"We played well but we were just unlucky. They used their chances well and we can blame ourselves. I'm happy we have qualified for the regional level and we will work hard to actively compete."
Elsewhere, Moi Forces Academy secured third place in the boys' competition after defeating Kajembe Secondary 2-0 in the playoff match.
In the girls' category, Kajembe Secondary were crowned Mombasa County champions following an emphatic 4-0 victory over Changamwe Secondary in the final.
